Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns from inadequate procedures to monitor furnace refractory lining: On or about April 27, 2004, employees working at and around the Mark V Pressure Pour Furnace number Five were exposed to the hazard of contact with molten metal in that the employer did not take steps to identify and prevent refractory failure that allowed molten metal to contact the cooling water in the flange cooling jacket. One feasible and acceptable method, among others, to correct this hazard is to: 1.Establish and follow an effective refractory maintenance program; 2.Monitor and document reduction in the vessel and inductor refractory on a regular and periodic basis; 3.Act to correct the hot spots that were identified. ***THIS ITEM REQUIRES DOCUMENTATION OF ABATEMENT***N
