Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: On 03/12/03 an employee was fatally injured when two pieces of 16" x 28.5' steel beams weighing approximately 3800 pounds rolled onto him. The steel was laid on a piece of lumber approximately 2.25" x 3.25" x 20" long. The piece of lumber was not long enough and was rotten on the inside. One feasible method of abatement is to use longer pieces of lumber and thicker pieces of lumber. Also store wood to be used for blocking in a dry location. Examine lumber before use to ensure it is not rotten and is suitable for use.
