Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: Employees were exposed to a fall hazard of approximately twenty-one (21) feet to the concrete foundation floor due to improper usage, incorrect number and placement of the Crosby Clips at the anchorage point and on the splice of the wire rope horizontal lifeline. This occurred on or about August 16, 2002 at the new Fine Arts Building of the Laredo Community College West end Washington Street, Laredo, Texas 78040. Among other methods, one feasible and acceptable abatement method to correct this hazard would be installation of a properly installed one-half (1/2) inch wire rope lifeline using three (3) Crosby Clips properly placed and with the saddles of the Crosby Clip placed on the live side of the one-half (1/2) inch wire rope horizontal lifeline, along with the use of a properly formed eye termination which is formed by incorporating a properly sized thimble, as required by the manufacturers recommended specifications and as specified in ANSI Z359.1 1992.
