Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by or caught between the hydraulic vehicle lifts and the concrete floor while installing new tires on vehicles: On or About 1/13/03 Location: Tire Bay Area (a) The #1 short rise pad lift, AMMCO Model B2250-no serial number, was missing a floor anchor bolt that fastens the lift frame to the concrete floor. (b) The #1 short rise pad lift, AMMCO Model B2250-no serial number, was missing the pins that attached the lift pads to the lift frame. (c) The #3 short rise pad lift, AMMCO Model B2250-no serial number, had a leaking and damaged hydraulic system. (d) The #3 short rise pad lift, AMMCO Model B2250-no serial number, was missing the pins that attached the lift pads to the lift frame. Among other methods, one feasible and acceptable abatement method to correct this hazard is as follow: 1. Develop and implement a standard operating procedure for the hydraulic lifts which includes daily and periodic inspections and a preventive maintenance program in accordance with the manufacturers specifications and as outlined in the American National Standard Institute(ANSI) Standard for Automotive Lifts- Safety Requirements for Operation, Inspection and Maintenance. 2. Instruct all employees on the safe operating procedures of all hydraulic lifts, including but not limited to the operation, maintenance, daily and periodic inspections.
