Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: Worksite located at T-Mobile Facility, 11830 Webb Chapel Road, Dallas, Texas 75234: On or about January 23, 2004, two employees who were unloading work surfaces, were exposed to being struck-by or crushed-by the load that weighed more than 1100 pounds. The employer did not provide a safe means to unload the work surfaces off the Bobtail truck. The 30-inch wide dolly could not be safely supported on a 26-inch wide lift gate.
