ABATEMENT DOCUMENTATION REQUIRED 12-60-2(a)(3) The employer did not provide safe work places and practices by elimination or reduction of existing or potential hazards; i.e., employees were exposed to trip hazards as there were two instances of concrete walking/working surfaces that were chipped and cracked. The damaged floor surface left an indention that was approximately 45 inches long, 15 inches wide, and 1.5 inches deep. Location:(1) Warehouse at the hangware belt area (2) Warehouse at the middle aisle Abatement Note: One feasible means of abatement is to resurface the damaged areas of the concrete floor.
