Minn. Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: Employees were exposed to fall hazards while working from a platform that was attached to the lifting carriage of a forklift. The securing device for the mid-rail chain was broken on the front side of the platform, and the two employees were not wearing approved-type harnesses and lanyards.
