Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1): of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to a striking/crushing hazard from falling concrete block; a) On or about 3/19 and 3/20/2002 at the south side, east end of the building - Employees were exposed to a striking/crushing hazard from concrete block that was set on top of a steel beam only held in place with masonry mix and when the trusses being erected fell, a secondary reaction pulled the block loose from the steel. This block placement was not addressed by the engineer/designer drawings for placement on the steel beam, nor was contact made with the engineer, as stated in the drawings, to address this omission:,"no other similar detail and/or the more costly item of conflict used". Among other methods. one feasible and acceptable abatement method to correct this hazard is to follow the engineer/designer drawing and general notes to address omissions and/or contact the engineer of record.
