Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by or caught in malfunctioning machinery and equipment due to damage not noted as there was no adequate program of periodic inspections nor an adequate maintenance program established for a hydraulic power press as evidenced by missing indicator/check valve, hydraulic fluid leaking around the base of the machine, and unlabeled tonnage capacity as specified by ANSI B11.2-1982 Instance A - Upon release of the button that initiated ram movement on a hydraulic press, the ram continued to drift. Instance B - Hydraulic press was not equipped with any tag or label to indicate the maximum tonnage. Instance C - Indicator/check valve for a hydraulic press to indicate ram pressure during operation was removed from the press. Instance D - The seals for the actuating ram on a hydraulic press were worn allowing hydraulic fluid to leak around the base of the machine. Instance E - The employer did not establish a hydraulic press maintenance program to correct malfunctioning components. Among other methods, a feasible and acceptable method of abatement to correct this hazard is to instruct and train operators on safe work procedures before allowing operation of any hydraulic power press covered by ANSI B11.2-1982 and ensure by adequate supervision that correct operating procedures are being followed. CORRECTED DURING INSPECTION - MACHINE WAS REMOVED FROM SERVICE.
