Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment that was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were not provided and trained on a safe working procedure to prevent exposure to explosion hazards while performing maintenance activities. December 26, 2002: Employees, performing maintenance on extrusion line #1, received 1st, 2nd and 3rd degree burns to the head and face after a build up of pressure caused the connection point between the secondary extruder and dynamic cooler to rupture, spraying flaming polystyrene over the immediate work area. Employees were not provided and trained on a safe working procedure that would have prevented the accident. Among feasible means of abatement would be to comply with the requirements of ANSI/SPI B151.7-1996, Plastics Extrusion Machines - Requirements for the Manufacture, Care, and Use.
