Text For Citation: 01 Item/Group: 001 Hazard: GUARDING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) A-Stamp Industries, Bryan, Ohio: As determined on or about June 10, 2002, during production operations on the Minster 500 ton part revolution mechanical power press, employees were using the "inch mode" to stamp parts during "station-to-station" hands-in-die press operations. During the use of the "inch mode" as described, the brake monitor did not monitor the performance of the brake, additional wear was placed on mechanical parts such as the brake and clutch and there was no single stroke/anti-repeat mechanism while operating in the "inch mode", which allowed the press to cycle continuously as long as the two control buttons were depressed. Among other methods, feasible and acceptable methods to correct this hazard would be to run production operations in another mode and cease the use of the "inch" mode during production operations, following the recommendations of the press manufacturer, which states: 1. Minster Press Company Manual, page 30: NEVER RUN PRODUCTION ON A PRESS WITH STROKING SELECTOR IN THE INCH MODE. INCHING IS A FUNCTION TO BE USED BY THE DIE SETTER FOR SET-UP OF DIES AND TOOLING, OR, FOR MECHANIC'S USE WHEN CONTROLLED INTERMITTENT MOTION OF THE SLIDE IS REQUIRED. Or the employer could follow the precepts of ANSI B11.1-2001, Safety Requirements for Mechanical Power Presses, which states: 2. ANSI B11.1, 2001, section 6.12.3.3.2, Inch: The INCH operating mode shall not be used as a production mode. Additional abatement information is required for this item.
