Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about October 19, 2001, at the jobsite located at 2342 West Bloomingdale Avenue in Chicago, Illinois, employees working from aerial lifts were exposed to injuries from falls due to the lack of (personal) fall protection systems required by the American National Standards Institute's standard for boom-supported elevated work platforms. (ANSI A92.5- 1992) Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
