Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Nevada Revised Statute 618.375(1): The employer failed to furnish employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm to his employees: The employer failed to ensure that employees were properly trained to operate a personnel lift: a.Outside and adjacent to the Receiving Department roll-up door, one employee, standing in the basket of a Genie Personnel Lift, Model PLC-30, failed to install outriggers before ascending to an estimated height of 23 to 25 feet. As a result, when he begin his descent, the lift swayed, fell over to the concrete surface and the employee sustained a severe back injury. Reference: Genie Personnel Lift Operating Instructions: Page 2 Caution, "Read and understand these instructions before operating the Genie Personnel Lift. Do not permit any to use the lift who does not understand the material in this document." Page 2 Set Up and back page item #2, "All outriggers must be installed and the base must be leveled before using the personnel lift." Among other methods, one feasible and acceptable abatement method to correct this hazard would be 1) Ensure that outriggers are attached prior to employees entering the basket and 2) Fully train each employee in accordance with ANSI A92.3 and Genie Personnel Lift Operating Instructions, prior to using the lift and any similar equipment purchased in the future.
