Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from falling equipment due to the non-retaining type bolts used to support the legs of the Mobile Rotary Impact Crusher. The bolts were subject falling out of the support legs due to equipment vibration: (a)At the APAC Asphalt Plant site - Decatur, Alabama - Brown Lenox Powerscreen Mobile Rotary Impact Crusher, Model 4228, Serial Number FR0066 - On or about 01/08/2002, non-retaining grade 8 threaded bolts were used in place of the manufacturer's specified retaining pins. Among other methods, one feasible and acceptable method of abatement would be to use and maintain retaining type pins as specified by the manufacturer.
