Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Facility Wide, on or about 11/1/02: Electric cart co#1101 used by employees to transport equipment, tools etc. had an inoperative horn. Electric cart is driven by employees from barn to barn or barn to another structure. ABATEMENT NOTE: Repair the horn and ensure that the operators check at the beginning of each shift the following items : carrier condition, inspect tires, warning devices, lights, battery, speed and directional controllers, brakes and steering mechanism per ASME/ANSI B56.8-1988 Safety Standard For Personnel and Burden Carriers, part II, Section 4.9.1 and Section 5.7.1 b)Bldg.#41, Maintenance Shop, on or about 12/30/02: A AMMCO automotive lift, model#82900, sn.P990229294, capacity 9000lbs.being used by emplopyees to lift pick-up trucks to perform maintenance/repair work was not being inspected as recommended by the lift manufacturer nor were records of inspection being maintained per American National Standard, ANSI/ALI ALOIM-2000. ABATEMENT NOTE: Ensure that the AMMCO automotive lift is being inspected per manufacturers recommended frequencies and that records of these inspections are being prepared per American National Standard, ANSI/ALI ALOIM-2000, Section 5.4. c)Facility Wide, on or about 11/6/02: 1996 Ford F350 dump truck with a sander attachment and operated on facility roadways did not have a reverse signal alarm. Motor vehicle had an obstructed view to the rear. d)Facility Wide, on or about 11/6/02: A Bobcat material handling equipment, co# E301 used by employees to pick-up and move different materials around facility grounds and which the operator has an obstructed view to the rear was not provided with a reverse signal alarm. e)Facility Wide, on or about 11/6/02: A Bobcat co# E318, material handling equipment used by employees to pick-up and move different materials around facility grounds and which the operator has an obstructed view to the rear was not provided with a reverse signal alarm. f)Facility Wide, on or about 11/1/02: A Caterpillar model 930 front end used by employees to pick-up and move different kinds and types of material around facility grounds was not provided with a horn. g)Facility Wide, on or about 10/31/02: A Ford tractor co.#302 with lifting forks and overhead operator protection was not provided with a seat belt. h)Facility Wide, on or about 10/31/02: A International tractor co.#317 with lifting forks and overhead operator protection was not provided with a seat belt.
