Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD : a)Installation Dept., on or about 12/10/01: A Campbell (1)ton capacity, pressure gripping lifter, below-the-hook lifting device being used by employees to lift and move sheet metal was not inspected prior to initial use nor according to the time frames of frequent and periodic inspections per American National Standard, ASME B30.20-1993. Employees exposed to a struck-by the load hazard. ABATEMENT NOTE: Ensure that each below-the-hook lifting device is inspected prior to initial use, also according to frequent and periodic inspection time frames with records maintained per American National Standard, ASME B30.20-1993, Chapter 20-1, Section 20-1.3, Table 1. b)Tool Room, on or about 11/27/01: A Coffing manually lever operated chain hoist, 1 1/2ton capacity, had a throat latch missing from load hook end. Employees use this equipment for lifting, pulling and tensioning type applications. Employees exposed to a struck-by the load hazard. ABATEMENT NOTE: Ensure that throat latches on hooks of manually lever operated chain hoists are maintained per American National Standard ASME HST-3M-1991, Section 3.4(b). c)Tool Room, on or about 11/27/01: A Milwaukee manually lever operated chain hoist, 3 ton capacity, had a throat latch missing from the load hook and the throat latch was inoperative on the suspension hook. Employees use this equipment for lifting, pulling and tensioning type applications. Employees exposed to a struck-by the load hazard. ABATEMENT NOTE: Ensure that throat latches on hooks of manually lever operated chain hoists are maintained per American National Standard ASME HST-3M-1991, Section 3.4(b).
