Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being crushed: a.Molding Department - The employer did not develop, implement and instruct employees in adequate procedures to prevent mold slippage for horizontal injection mold machines, nor provide for periodic and regular inspections to ensure proper mold attachment. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement would be to follow the guidelines in ANSI/SPI (American National Standard Institute) B151.1-1997: Horizontal Injection Molding Machines - Safety Requirements for Manufacture, Care, and Use. Adequate procedures to ensure mold slippage does not occur include but are not limited to: 1.Develop, implement and instruct employees on adequate criteria and procedures to determine the minimum number of clamps, appropriate bolt tightness and clamp assembly installation procedures. 2.Develop, implement and instruct employees on adequate criteria and procedures to identify and remove damaged equipment and tools such as but not limited to clamps, washers, bolts, platens, air impact wrenches, torque wrenches and universal swivel sockets. 3.Conduct an analysis and develop procedures to assure safe matching of molds and platens. 4.Develop, implement and instruct employees on adequate procedures so that clamps are installed to prevent the mold from falling out from between the upper and lower clamps. 5.Develop, implement and instruct employees on adequate procedures and implementation of mold blocking to prevent slippage. 6.Develop, implement and instruct employees on the adequate procedures for cleaning the horizontal injection mold machine platen and mold back plate. 7.Conduct a periodic inspection and evaluation of clamp installation for securing molds to horizontal injection mold machines.
