Code of Virginia-Section 40.1-51.1.(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards because the two hand controls on Lawson Pacemaker II shear (serial #CA1795) were not wired for concurrent operation. Among other methods, one feasible and acceptable abatement method to correct this hazard is to properly wire the controls for concurrent operation.
