Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: (a)On or about 02/27/03, at 18933 and 18815 Aldine Westfield, employees were exposed to the hazard of being struck by and/or crushed by plate or coil steel because below-the-hook lifting devices, including but not limited to: plate hooks, plate hooks with chain attachments, spreader bars, and roll lifting devices, were not load tested and no load testing documentation was available. Plate hooks were not labeled with rated load information, and no program of frequent and periodic inspection of the below-the-hook lifting devices had been established to identify hazardous deficiencies in the lifting devices. Feasible and useful abatement methods to correct these hazards include, but are not limited to load testing and inspecting all below-the-hook lifting devices in accordance with ANSI B 30.20 - 1993 Below-The-Hook Lifting Devices. *** Certification and Documentation of Abatement Required ***
