Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling and crushing hazards. a)1st floor of tannery - On or about 3/21/02 - The hazard of falling into an open elevator hoistway, approximately 12 feet to the bottom of the pit, basement, or being caught between the ascending car platform and the wall due to the inoperable electro mechanical interlock which allowed operation of the freight elevator with the hoistway door in the open position. Feasible and acceptable means of abatement are: Repair the electro mechanical interlocks on the 1st floor hoistway door. This Method of abatement is listed in the following reference: ASME A17.1, 1996: Safety Code for Elevators and Escalators which describes the proper adjustments and design of combination mechanical locks and electric contacts. ABATEMENT DOCUMENTATION AND CERTIFICATION IS REQUIRED
