Text For Citation: 01 Item/Group: 001 Hazard: CONFINED
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: Employees were exposed to the hazards of being overcome, engulfed, or trapped in confined spaces, such as, but not limited to, sanitary sewers and manholes, due to not following confined space entry procedures in the following situation: a.On or about April 16, 2001, an employee entered an active sanitary sewer manhole on the Cincinnati-Dayton Road jobsite without the following: 1.Chest or full body safety harness. 2.Retrieval line attached to a safety harness. 3.The other end of the retrieval line attached to a mechanical device or fixed point outside the confined space. 4.A tripod and mechanical device for non-entry rescue or documented entry rescue procedures and services. 5.Confined space entry permit to document the air test results and acceptable entry conditions, as well as, identify who the authorized entrant, attendant, and supervisor are and what equipment such as personal protective equipment, testing equipment, communication equipment, alarm system, and rescue equipment may be required. 6. A standby person/attendant trained on the signs and sypmtoms of someone being overcome or over-exposed to different substances. Among other methods, one feasible method to correct the hazards is to implement a comprehensive confined space entry program.
