Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: 933 Mulberry Street, Kansas City, MO 64101; Employee(s) working at or near the Femco CNC Lathe were exposed to struck by hazards. The employer did not ensure that retaining covers on the Femco CNC Lathes were of such material and thickness as to withstand the high impact forces or weight of a workpiece that might strike it, as required in ANSI B11.6-1984, American National Standard for the construction, care and use of lathes. On or about August 30, 2001 an employee was fatally injured when he was struck by the retaining cover of the Femco lathe which separated from the machine after a rotating workpiece came unclamped from the work holding device. One feasible and acceptable abatement method, among others is to install retaining covers that will contain any workpiece should it fall or fly out from the work-holding device, in accordance with requirements of ANSI B11.6-1984. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
