Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: The employer failed to provide written safety procedures along with hazard recognition and training in safe procedures for loading and unloading rail cars. The procedures need to include instructions for coupling and uncoupling of rail cars. On May 17, 2001, an employee was fatally injured when attempting to connect two rail cars.
