Minn. Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: Ladders placed in the interior mechanical shaft intended for access and egress had fall exposures of approximately 140' from floor eleven to floor two. Employees were not provided with a positive means of fall protection such as side rails, floor hole coverings or other effective alternatives.
