Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Serious Citation 1, item 1 Title 40.1-51.1(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing of likely to cause death or serious physical harm to employees were exposed to a crushing hazard when unbraced granite slabs (approximately ten feet by six feet and weighing approximately seven hundred pounds each) were being unloaded from a truck trailer. An employee was killed and a second injured when the granite slabs shifted during unloading procedures (the shipping package was removed while the slabs were still on the truck contrary to company unloading procedures - when the shippers bracing was also removed, the granite slabs shifted, striking the employees). One feasible and acceptable abatement method among others to correct this hazard is to comply with the company unloading procedures which require that the granite slabs remain inside the shipping package while being unloaded . Also there was no secondary bracing (either a wood cross bracing or the boom from the fork lift) to prevent the slab(s) from moving, after the shipping brace is removed and before the air clamps were installed by the employees. On April 16, 2001 one of the three employee was struck by these materials and sustained fatal injuries.
