Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of automotive lift(s) falling while working under or around lift(s): a) Garage bay area, north side of garage, 3rd lift from the west end - On or about 02/06/01, the employer did not ensure that automatic locking devices on the ROTARY - two piston, in- ground, automotive lift m/n DT028H MKV1 was maintained in an operable condition for the continued safe operation of the lift in accordance with paragraph 7.1.2. of the American National Standard Institute (ANSI) Publication B153.1-1990. b) Garage bay area, north side of garage, 5th lift from the west end - On or about 02/06/01, ROTARY - single piston, in-ground, automotive lift (no m/n avail) was maintained in an operable condition for the continued safe operation of the lift in accordance with paragraph 7.1.2. of the American National Standard Institute (ANSI) Publication B153.1-1990. c) Garage bay area, north side of garage, 6th lift from the west end - On or about 02/06/01,ROTARY - single piston, in-ground, automotive lift (no m/n avail) was maintained in an operable condition for the continued safe operation of the lift in accordance with paragraph 7.1.2. of the American National Standard Institute (ANSI) Publication B153.1-1990. d) Garage bay area, south side of garage, 2nd lift from the east end - On or about 02/06/01, ROTARY - single piston, in-ground, automotive lift (no m/n avail) was maintained in an operable condition for the continued safe operation of the lift in accordance with paragraph 7.1.2. of the American National Standard Institute (ANSI) Publication B153.1-1990. e) Garage bay area, north side of garage, 3rd lift from the west end - On or about 02/06/01, the employer did not ensure that the ROTARY - two piston, in-ground, automotive lift m/n DT028H MKV1 was maintained in an operable condition for the continued safe operation of the lift in accordance with paragraph 7.1.2. of the American National Standard Institute (ANSI) Publication B153.1-1990 due to a internal condition with the hydraulics which caused the lift to rise and descend erratically during operation. Among other methods, several feasible and acceptable abatement methods to correct these hazards include: (1) Cleaning and maintaining safety devices to ensure proper function; (2) Establishing a periodic inspection procedure as recommended by the manufacturer to ensure the safe operation of the lifts; (3) Posting lift operation instructions and instructing all employees in the safe use and operation of the automotive lifts and their safety devices and ensure their routine use; (4) Installing permanent marking on the lifts to show the manufacturer's name, rated load capacity, model and serial number in accordance with standards referenced in American National Standards Institute (ANSI) B153.1, 1990 and the manufacturer's installation, operation, and service manual.
