Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to: a. On or about October 3, 2001, at the above addressed jobsite, employees were exposed to a crushing hazard by removing a temporary steel support beam in an unsafe manner. The beam was intended to be pushed and rolled down a 4x4 piece of lumber, which had been placed on an angle between where the beam was located and the ground. Among feasible means of abatement would be to comply with the SAFE PRACTICES FOR ERECTING AND DISMANTLING OF FRAME SHORING as recommended by the Scaffolding, Shoring and Forming Institute, Inc. and Scaffold Industry Association. In particular, III. Guidelines for Dismantling Shoring, D. LOWER SHORING COMPONENTS in a safe manner.NTS
