Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple injuries or death from a tipping over hazard using an oversized platform on a rough terrain forklift truck: (a) Employees were exposed to multiple injuries and/or death hazards created by the use of elevated work platform which were not designed, constructed, and/or used in accordance with the acceptable requirements of the American National Safety Standard for Rough Terrain forklift Trucks, ASME/ANSI B56.6a-1994 and other acceptable industry practices. For example: The platform used did not meet the following design and use requirements of sections 8.25 and 5.15 of ASME/ANSI B56.6a-1994 in the following areas: (1) Platform dimensions exceeded in width more than 10 inches greater than the overall width of the truck on either side, (2) Did not have a maximum work load and empty weight information prominently marked on the platform, (3) The platform was not lowered when moved for positioning, (4) A trained operator was not available to operate the controls when the platform was not equipped with controls. The above conditions were observed when an employee was installing roof sheeting and facia at the roof eve of the building front in a 10 foot, 8 inch wide platform attached to a Sky Lift rough terrain fork lift truck with a tire width of 8 feet. Among other methods, feasible and acceptable means to correct these hazards are to use safer alternative methods such as a scaffold or use a platform approved by the manufacturer meeting the design, manufacture, training, and use requirements of AMSE/ANSI B56.6a-1994 for all attachments on rough terrain forklifts. No Abatement Certification or Abatement Documentation required.tion
