Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees; specifically: Chain device used for opening and closing the overhead door, used to provide access to a storage area from the alley way, was not operational, and the door tension control was not properly maintained. At the time of this inspection, the door was being held open by a 2x4 piece of lumber which had been wedged under it.er
