Text For Citation: 01 Item/Group: 001 Hazard: GUARDING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Hazards created by the use of elevated work platforms which were not designed, constructed and/or used in accordance with the acceptable requirements of the American National Safety Standard for Rough Terrain Forklift Trucks, ASME B56.6-1992. The personnel platform and its use did not meet the design requirements of that standard in the following areas: (1) Modifications and additions, such as a personnel platform, did not have the manufacturer's prior written approval. (2) Restraining means such as a guardrail or a means for securing personnel such as a body belt and lanyard were not installed. (3) A trained operator was not in position to control the rough terrain forklift. These conditions existed when two employees were installing facia board while standing on a stack of plywood that was on the forks of a forklift. Among other methods, one feasible and acceptable abatement method to correct these hazards is to assure: That the design, manufacture and use of all attachments for Rough Terrain Forklift Trucks are in accordance with the most current American National Standard, B56.6 (presently B56.6-1992), and/or has the prior approval of the lift truck manufacturer.
