Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by a powered industrial truck in the event of turnover: A) Near Herman motor area - The employer did not ensure that an employee who was operating a Yale Forklift, number 2116, was wearing the seat belt. B) Main aisle by maintenance shop - The employer did not ensure that an employee who was operating a Yale Forklift, number 2117, was wearing the seat belt. C) Main aisle by mold cell - The employer did not ensure that an employee who was operating forklift, number 28, was wearing the seatbelt. Among other methods, one feasible and acceptable method to correct this hazard would be to instruct employees to utilize seatbelts during operation of powered industrial trucks and implement a company disciplinary/enforcement policy to ensure compliance.
