Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: On or about July 11, 2002, and at times prior thereto, at the Burkhart Well site #1R, employees were exposed to the hazard of being struck by pressurized flow line of pressurized gas due to the following: Failure to secure and/or restrain the flowline; failure to ensure piping, hoses, valves and other fittings were not operated at pressures greater than their rated working pressure; and failure to ensure that piping, hoses, valves and other fittings were maintained in good operating condition. One feasible and acceptable abatement including, but not limited to, the following, is to have piping, hoses, and pressure relief devices designed to meet the requirements of American Petroleum Institute, Recommended Practices for Occupational Safety for Oil and Gas Well Drilling and Servicing Operation.
