Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Copeland Enterprises, Inc. at 904 S. Lipan St. Denver, CO: Proper work procedures were not in place to ensure that all concrete forms were restrained during the form stripping process. Employees were exposed to crushing injuries while stripping concrete forms. Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following: ANSI A10.9-1970,American National Standards Institute-safety requirements for concrete construction and masonry work, Section 6.4.5 "Vertical forms being raised or removed in sections shall not be released until adequately braced or secured".
