Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by/crushing hazard: (a) Employees were exposed to a struck by/crushing hazard while working on Nabors Drilling Rig No. 414, on Well No. GM 443-33, at approximately Mile Marker 3.5, County Road 215, Parachute, CO 81625 on November 4, 2001. The employer, Nabors Drilling USA, Inc., failed to adequately secure this drilling rig's blow out preventer (BOP), which fell over and crushed the leg of an employee. Among other methods, one feasible and acceptable method to correct this hazard is to require that the BOP be adequately secured at all times using as many tie-off points as necessary to prevent it from falling over during the procedure referred to as "Nipple Down and Set Out".
