Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the brake handle on a drawworks mechanism: a) On or about June 14th, 2001, the driller on rig #1 received serious injuries after being struck by the violent movement of the brake handle that controls the friction brake on the drawworks machine. Prior to this accident, the brake linkage was adjusted to address previous brake problems. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that brake systems on the drawworks are maintained according to the manufacturer's recommendations as found in section 8.4.6 in the American Petroleum Industry (API) publication titled, Recommended Practice 54, second edition, 1992.
