Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by a moving elevator car: On or about September 11, 2000 at 1 West Flat Irons Circle; Suite 224 Broomfield, CO 80021 an employee sustained a fatal injury when he was trapped between an elevator car and the floor. The employee was engaged in the installation of a Dover Cimmarron Hydraulic Elevator (2 stop). Energy isolation procedures were not put into place allowing the car to descend and trap the employee. The car crushed the employee in between the first floor surface and the car itself. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that the elevator car is not able to move at times when employees must place their bodies in an exposed area to gain access to the pit or to perform installation operations. Employees may utilize lock out procedures, car blocking procedures, early wiring of the pit stop switch, and/or required use of the temporary run buttons.
