Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: a)On March 15, 2001, a derrick hand was fatally injured when he fell approximately 108 feet with the derrick after guywire anchors "deadman" pulled loose causing the derrick to collapse. The employer id not protect employee(s) from recognized hazards of possible derrick collapse by verified anchor capacity before attaching guywires to the anchors. b)On March 15, 2001, a derrick hand was fatally injured when he fell approximately 108 feet with the derrick after guywire anchors "deadman" pulled loose causing the derrick to collapse.
