Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Southwestern Wire, Inc. employees were required to light the sand furnace #1 burner with a flaming piece of paper attached to a three foot cardboard tube. This exposed employees to the recognized hazard of burns from flashback of igniting gas. A feasible and acceptable method to correct this hazard would be: Install a pilot light device or use a non-flammable extension rod for lighting the burner in lieu of the cardboard tube. ***Certification and Documentation of Abatement Required***on
