Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by falling load from a chain hoist: Among other methods, one feasible and acceptable method to correct this hazard is to use an approved lifting sling or device. a) On or about September 19, 2001 at the number two coker located at Solutia, Inc. plant in Decatur AL, a different contractor's employees were attempting to position a large boiler tube sheet with the use of a Jet L-90 two ton overhead chain host. The chain hoist load line on the east end was passed through one of the 180 degree tubes and the hook was hooked back to the load line this contractor's employees, therefore using the chain of the hoist as a sling around the load to lift it. In addition, a come along was hooked to the load line of the hoist by the other contractor's employees to move the load laterally, thereby exposing Respondent's employees to death or serious physical harm. Both the Jet L-90 operators manual and A.N.S.I. B30.16 prohibit this practice.
