Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 09-19-00 two rough necks were walking under a 12 foot (2.6m)high staircase leading to the rig floor. Four staircases were in use, but not secured to the sub structure by pins or bolts for over one day. One staircase fell when a wire rope from a winch truck caught the handrail while straightening the A-frame. Two rough necks were walking under it and one rough neck was struck in the head killing him when the staircase fell to the ground. One method of abatement among others is to secure all staircases immediately when it is put in place and to prevent anyone from being directly underneath overhead operations without protection from falling objects while rigging up.ging
