Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Mayers Lease, Well No. 2, near San Angelo, Texas: On or about 09 June 2000, the main disconnect providing power to the electrical control panel of the oil well pumping unit was not locked-out. The employee was exposed to a struck-by-moving parts (i.e. counterweights) hazard and a fall-from-elevation hazard while he was standing on top of the gearbox servicing the brakes while the unit was pumping. One feasible and accepted method would be to use a lock-out program to protect employees from energy sources and to require the use of personnel fall protection when exposed to a fall-from-elevation hazard.om-elevation
