Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees. On Patterson Drilling Rig #55, located at KCS Medallion Shugart 19 W#3, the kelly hose was not securely fastened during the rig down process. Not securing the kelly hose is contrary to API RP 54 - 8.7.7 which states: The pump end of the rotary hose should be securely fastened to the derrick or mast by a cable or by a chain clamped to the hose and to the derrick or mast leg. The swivel end of the hose should be secured by a similar cable or chain, with the other end of the cable or chain affixed to the swivel.
