Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 6/29/00 in the new addition of bus maintenance, garage employees using a JLG Scissor Lift Model 2033, SN 200015069 were exposed to the hazard of tipping over while driving onto an unsupported plywood floor hole cover that was under a canvas tarp. Feasible abatement methods include, but are not limited to: Ensure equipment maintains a safe distance from any obstacles, debris, drop-offs, holes, depressions, ramps and other hazards to ensure safe elevated travel as per ANSI A92.6, 8.10.15 (2). Implement a training program to ensure that employees are aware of the proper methods to be used to operate scissor lifts such as those described in ANSI/SIA A92.6 - 1990.
