Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns and smoke inhalation from fire due to a delay of employees being evacuated due to lack of training. a) A fire occurred on Conveyer F-35, Col. U-5. 1. The alarm system changed in that the warden phone sound changed. 2. The anunciator in the control room did not work. 3. The (key) alarm system did not activate. This system change had been in efect for approximately 6 months without of training employees on the important important changes. Among other methods one feasible and acceptable abatement method to correct this hazard is to provide employees training on the new "Fire Emergency Procedures", and ensure that the alarm system is working. ABATEMENT CERTICATION AND DOCUMENTATION ARE REQUIRED within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
