Title 40.1-51.1(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from the recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) At this site (Community First Bank, 1646 Graves Mill Rd, Lynchburg, VA), Ragnarock failed to ensure that all protruding drive pins throughout the work area were guarded to the hazard of impalement. Most of the drive pins ranged in height between approximately 12 and 18 inches above the ground. There were approximately 50-70 drive pins at this site. "One feasible and acceptable abatement method, among others, to correct this hazard is to cap the drive pins."
