Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls from elevation: a.The cables (wire ropes) on the Sponco Aerial Ladder Model 78 were not inspected and replaced at the time of the first observance of defects such as rust, corrosion, wear, crushing, pinching or broken strands as required by the Sponco Installation, Operation, Maintenance and Repair Parts Manual(Section VII). b.The cables (wire ropes) on the Sponco Aerial Ladder Model 78 were not replaced annually regardless of their condition as required by the Sponco Installation, Operation, Maintenance and Repair Parts Manual (Section X). c.The main extension cable (wire rope) on the Sponco Aerial Ladder Model 78 was not replaced with a 7x19, 5/16 inch galvanized aircraft grade wire rope as recommended by the manufacturer, Sponco Mfg., Inc.. Feasible abatement methods could include but are not limited to: 1.Develop and implement a comprehensive inspection program for the Sponco Aerial Ladder. Document inspections on a checklist. Refer to the Sponco Installation, Operation, Maintenance and Repair Parts Manual Sections VI, VII, VIII and IX. 2.Replace the cables (wire ropes) annually as required by the Sponco Installation, Operation, Maintenance and Repair Parts Manual (Section X). 3.Inspect the cables (wire ropes) for defects such as rust, corrosion, abrasion, rope stretch, reduction in diameter, kinks, broken strands, bird caging, heat damage, protruding core, damaged end attachments, peening and scrubbing and replace if defects are observed. 4.Replace the cables (wire ropes) with cables (wire ropes) recommended by the manufacturer. 5.Provide training on procedures to follow in cable (wire rope) inspection for the employees operating and maintaining the aerial ladder.
