Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) a fall from elevation hazard exist when the manufactures recommendations for the safe operation of the Prime Mover OE-15 C orderpicker/forklift regarding the use of the safety chain at the operators compartment and the use of a safety belt when the operator moves from the operators compartment to the platform were not followed. An employee/operator off loading boxes of chairs from the platform on to a steel storage rack located in isle 22 of the warehouse, was exposed to a fall hazard of 15 feet to the concrete floor and the 4 inch by 4 inch by 1/4 inch steel guide rail at the floor level. The latch on this chain was broken and not latchable or lockable. "ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO FOLLOW THE MANUFACTURES RECOMMENDATIONS AS WRITTEN IN THE OPERATING/MAINTENANCE/PARTS MANUAL UNDER, OPERATOR CARE OF THE TRUCK ITEM D ISSUED IN JANUARY OF 1983."
