Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following condition(s): a) The employer did not provide suitable information and training for employees lowering the gantry assembly on a Manitowoc 3900W crawler crane and thereby did expose them to the hazard of being struck by falling equipment. Note : Among other methods, some feasible and acceptable methods to correct this problem are : 1) Train the crane operators to never attempt to lower the gantry assembly with employees on the roof of the crane. 2) Train the crane operators to never attempt to lower the gantry assembly without using the gantry lifting device. 3) Train the crane operators to never attempt to lower the gantry assembly without having pinned the equalizer to the boom but. 4) Insure that all crane operators have read the current operators manual for the specific machine they are using and that they understand all the manufactures instructions and safety warnings. 5) Train all employees to never work under any suspended loads or unstable equipment. b) The employer did not provide and maintain the equipment needed by employees to safely lower the gantry assembly on a Manitowoc #3900W crawler crane and thereby did expose them to the hazard of being struck by falling equipment. Note: Among other methods, some feasible and acceptable methods to correct this problem are : 1) Provide a current copy of the Operators Manual for a Model 3900W crawler crane chained to the wall of the machines cab as specified by the manufacturer. 2) Provide the orginal manufactures equipment needed to secure the equalizer to the boom but. 3) Provide the maintenance needed to insure that the gantry lifting device is working properly and that the equipments hydrolic system is in good repair and not leaking.
