Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1)of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to : a) Being crushed by the overhead guard on a Clark forklift (model GPX300E; serial CRX 2302CE-0033-927FB) as he drove the machine on the south side of the combination boiler on 11-22-2000. As he made a turn to pick up a dumpster, the rear wheels on the right side hit a 15 degree slope around a drain in the concrete. The rear wheels skidded causing the lift to roll to the right. The employee attempted to jump clear of the over turning forklift but was pinned by the overhead guard. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to vigorously enforce the wearing of seat belts while driving the forklitft.
