Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls of up to 10 ft. from the inadequately guarded access side of a "cage type" workplatform raised in the air by the forks of a powered industrial truck. a) On or about 9/26/99, in the west end of the machine shop bay: Employee was exposed to a fall of up to 10 ft. from a "cage type" workplatform raised in the air by the forks of the yellow Clark forklift truck, model # C500Y80, serial # Y6852172342. Only a single chain, which was 36 inches above the floor of the workplatform, was fastened across the access side of the platform opposite the mast and above the tips of the forks. No midrail or midchain was installed at a level of approx. 21 inches above the floor of the workplatform. b) On or about 8/16/99, in the shop by the King Vertical Mill: Employee was exposed to a fall of up to 6 ft. from a "cage type" workplatform raised in the air by the forks of the yellow Clark forklift truck, model # C500Y80, serial # Y6852172342. No standard guardrails or chains were installed across the access side of the platform opposite the mast and above the tips of the forks at levels of approx. 42 inches, and 21 inches respectively, above the floor of the workplatform. Among others, a feasible method to eliminate or materially reduce this hazard is to install a hinged or removable guardrail, or chains at the access opening as required by ASME B56.1- 1993.
